留学準備：オーストラリアにペンタサ(Mesalazine)を持ち込む/郵送する方法

 0. 結論
持ち込み：
処方箋があれば3ヶ月分までOK。

郵送：
Personal Importation Scheme での輸入(郵送)場合、処方箋を書くのがオーストラリアの登録医である必要あり。

3年半の滞在となる今回は、日本で薬を処方して郵送するのは諦めて現地調達することにしました。僕の場合、海外転出の手続きを行って現地で薬を調達する方が、住民税と国民健康保険を払い続けた上で薬をオーストラリアへ郵送するよりも経済的。


1. 持ち込み

医薬品の持ち込みにあたっては Department of Health, The Australian Customs Service, The Australian Quarantine and Inspection Service が関係部署で、それぞれに確認が必要。

薬を持ち込む際の注意点。
持ち込む薬が規制対象となっていないこと
自分用の処方であること、薬の成分が確認できる処方箋を準備しておく（当然英語の物）
持ち込める量は最大3ヶ月分の処方量
旅行者、個人輸入については、OCSEH と TGA の特例も規制の対象となる

"Personal Importation Scheme

Under the Personal Importation Scheme you may import a 3 month supply (at the maximum dose recommended by the manufacturer) of unapproved therapeutic goods into Australia in any one importation without any approval required by the TGA provided that:

    * the goods are for your own treatment or the treatment of your immediate family; and
    * you do not supply (sell or give) the medicine to any other person; and
    * where possible, you keep the medicines or medical devices in their original packaging with any dispensing labels intact; and
    * the goods are not restricted under Australian Customs controls or quarantine rules and the goods do not contain a controlled substance (see: Import & export of controlled substances); and
    * the goods are not injections that contain material of human or animal origin (except insulin); and
    * the total quantity of the goods imported within a 12 month period does not exceed 15 months supply of the goods (for medicines, at the maximum dose recommended by the manufacturer); and
    * if the goods are medicines in Schedule 4 or 8 of the Poisons Standard a prescription from a registered medical practitioner is held for the medicines."

Mesalazine が Schedule 4 に入ってる。よって Personal Importation Scheme の要求事項である Registered medical practitioner 処方箋が必要。"Registerd medical practitioner" の定義がハッキリとしないが、Webの実例を確認する限り日本のドクターが作成した英語の処方箋 or処方箋＋英訳でOKな模様。

Schedule 4 の定義
"Prescription Only Medicine, or Prescription Animal Remedy – Substances, the use or supply of which should be by or on the order of persons permitted by State or Territory legislation to prescribe and should be available from a pharmacist on prescription."

Further information is available form the Therapeutic Goods Administration in relation to personal importation under the Therapeutic Goods Regulations 1990."

Australian registered doctor の処方箋が必要。

なにこれ？少なくとも、Mesalazine の場合は Import Authorization を得る必要はない。

上記内服薬の場合と記述が矛盾する。
少なくとも、Mesalazine は Custom, Quarantine 的にOK で、Import Authorization を得る必要なし。Personal Importation Scheme で要求される処方箋(英語のもの)さえ用意すればOK、ということかな。

これまで10年間メインバンクに三菱東京UFJを使ってきたが、海外からも利用することを
考えた時に Citibank、新生銀行あたりが無難に見えてきた。

新生銀行が意外に使える。

生活圏内にATMがあること、留学生は口座維持手数料が無料なことから
ANZ の ANZ Access Advantage と Online Saver Account という口座を開設することにした。
（両方とも日本にいながら開設可能）

ANZ の日本支店で、現地口座開設媒介サービスをやってるが、本国のサイトの
ANZ Online Applications から直接申し込みできる。

当初、Citibank か ANZ で迷っていたけれど、生活圏内にATMが無く...。
日本の Citibank が Citibank Global Transfers の対象外になっていたことも残念。

留学準備：日本国内の引越し UPDATED 1

2000年3月から11年住んだ東京を離れ、北海道に住処を移す、その準備。
単身長距離、ベッドがあって、荷物も多数、というなら日通の単身パックXがお買い得か。
JRのコンテナを１つ借り切って、それに入るだけ詰め込めるプラン。

ヨドバシカメラ経由で申し込むとヨドバシのカードで7%ポイント還元、なんて割引も。
問い合わせてみたところ、単身パックXは↑の割引の対象外でした。

留学準備：パース（西オーストラリア）までの航空券を手配する時の備忘録 UPDATED1

1. 日本から Perth への直行便はない。
2. 2011年9月くらいに成田 → Perth に行くんだと楽天トラベルで一番安い航路のチケットを購入できる。
3. 今回は Malaysia Airline の Kuala Lumpur 経由の便でパースに入る。
   Jet Star だと Perth 着が深夜の便のみ
4. Kuala Lumpur International Airport のトランジット、空港から無料シャトルバスが出てる Concordo INN の Early Fliers Package を使って予約をする。（1泊朝食付 5000円くらい）

追記 5/4/2011
15RMでエコノミークラスのシートをファーストorビジネスにアップグレードできるかもしれないプランがあった。シートに空きがあればアップグレードできて、アップグレードできなかった場合は15RMは返金されるみたい。これはチャレンジ。

7/24/2011追記：アセスメントレベル１の日本は、基本的に、資産証明の提出を要求はされない模様。Student VISA 申請のところで、自分で資産は十分にある、と宣言するだけでOK。日本のオーストラリア大使館に問い合わせたところ、ビザ発給のタイミングで要求されなければ不要（入国時に提示を求められることはない）とのこと。

留学準備：慢性疾患をかかえつつオーストラリアに留学するには

留学先はオーストラリア。
クローン病を抱えつつオーストラリアに留学するには。

オーストラリアに学位取得目的で留学をすることになると Student VISA をとることになります。んで、Student VISA 発給のためには、Overseas Student Health Cover (OSHC) という保険に入る必要があります(強制加入)。この保険でクローン病の治療のための費用をカバーできるかを考えてみました。

薬をオーストラリアで調達する場合
前提
1. 海外転出届けを出して、税金、国民健康保険を払わない
2. 保険加入前から煩っている既往症については１年間の待機期間がある
3. 投薬費用の補助の上限が年間$300、1回あたりの補助の上限は$50。
費用
診察料 ＋ 投薬量（ペンタサ1日4gの処方だとして、$330/月、$3960/年）
1年目： $3960 + 診察料
2年目以降：$3660 （診察料はOSHC にてカバー）


薬を日本で調達して郵送する場合
前提
1. 退職した年、その次の年はそれまで働いていた会社の保険の任意継続被保険者となる
2. ↑の次の年からは日本に残る家族の扶養家族になる
費用
1年目：住民税、今働いている会社の任意継続被保険者になる費用
2年目以降：国内に残る家族の扶養家族としてかかる住民税、保険料、特定疾患更新のための費用（日本への旅費）


海外転出して住民税や国民健康保険料を節約するのと、上記費用の比較で決めることになるのかな。ただし、海外転出をしちゃった場合、体調が悪くて緊急帰国、なんてときに面倒なことになるのかもね。今回は日本から薬を輸出する方向で進めてみようかと。
